Job Title: Senior Care Coordinator

">About the Role

We are seeking an experienced and compassionate individual to join our team as a Senior Care Coordinator. In this role, you will have the opportunity to make a meaningful difference in the lives of others by providing high-quality care and support to clients.

  • You will be responsible for conducting initial client intake and screening assessments, developing and reviewing individualized care plans, and providing quality care based on clients' needs.

  • You will also engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team and as required.

  • You will spend your time on the road completing scheduled visits, interacting one-on-one with clients within their homes, supporting with daily activities, and providing care in accordance with their individualized Care Plan.

Requirements

To be successful in this role, you will need:

  • Certificate III Aged Care, Individual Support or an equivalent qualification.

  • A current Australian Driver's License (Green Provisional minimum).

  • A roadworthy and insured vehicle to use for work.

  • Experience with technology, including smartphones, tablets, and laptops.

  • Confidence to satisfy the relevant probity checks required by legislation or policies.
