Case Coordinator Registered Nurse

Description

We are seeking a skilled Case Coordinator Registered Nurse to lead a clinical team in delivering integrated community care to clients with complex acute and aged care needs.

The ideal candidate will have minimum five years post-graduate nursing experience with demonstrated clinical leadership and resource coordination skills.

Key responsibilities include leading and supporting a dedicated clinical team, coordinating resources to deliver person-centred care, and driving effective communication through team meetings and thorough documentation.

You will work closely with the Clinical Nurse Manager to achieve service delivery goals and uphold our commitment to excellence.

This role operates Monday to Friday, between 7am and 4pm.

Required Skills and Qualifications
  • Bachelor of Nursing with current AHPRA registration
  • Minimum five years post-graduate nursing experience with demonstrated clinical leadership and resource coordination skills
  • Proven ability to meet KPIs and make innovative decisions that support quality client care
  • Skilled in mentoring staff and fostering professional development
  • Excellent communication, negotiation, and problem-solving skills
