Clinical Care Coordinator Role

This pivotal position involves coordinating clinical services to support clients in their own home, requiring a dedicated and compassionate individual.

The primary point of contact will ensure services are delivered efficiently and effectively by supporting clients and staff.

Key Responsibilities:

  • Coordinate daily care for new and existing clients
  • Accept referrals and ensure client information is available for service delivery within required timeframes
  • Identify eligible clients for other Government funding streams and liaise with referrers for transfer of care

Requirements:

  • Bachelor of Nursing degree or equivalent qualification
  • Experience in acute nursing and community nursing
  • Full driver's licence and ability to work over 7-day roster
  • Previous experience managing service contracts and staff, including strong problem-solving and time management skills
  • Effective communication and computer skills
