About the Role

We are seeking a highly skilled and dedicated Clinical Nurse Specialist to join our dynamic team at integratedliving. As a key member of our clinical team, you will play a vital role in delivering high-quality patient care and promoting health outcomes for our clients.

Key Responsibilities
  • Provide health promotion and education to clients and carers, empowering them to take control of their health and well-being.
  • Promote and deliver our award-winning Wellness for Independence programs, both in-person and virtually, to support clients in achieving their goals.
  • Monitor and promote health outcomes via Telehealth, ensuring timely and effective care for our clients.
  • Deliver direct care services across a range of funding programs, including wound care, diabetes care, medication management, stoma care, and catheter care.
  • Assess, develop, and evaluate nursing care for all clients through a collaborative and person-centered approach.
  • Work with clients to develop and achieve client care plans that are goal-driven and client-focused.
  • Assist clients in becoming independent in managing their health needs through a Wellness and Reablement approach, in conjunction with Allied Health clinicians.
  • Demonstrate a strong understanding of community nursing and the principles of primary health care.
  • Hold a Division One Registered Nurse qualification with the Australian Health Practitioners' Regulation Authority (AHPRA).
