**Care Coordinator Role**

We are seeking a motivated and compassionate individual to join our team as a Care Coordinator.

The successful candidate will work collaboratively with our nursing team to provide high-quality patient care.

Key responsibilities include:

  • Assisting with daily tasks and activities for patients

  • Maintaining accurate records and communicating effectively with the healthcare team

To be successful in this role, you will possess excellent communication and interpersonal skills, be able to work effectively in a fast-paced environment, and have a strong commitment to providing quality patient care.

A Certificate III in Health Service Assistance or similar qualification is required. Previous experience in a similar role is also highly regarded.
