Advanced Care Planning and Palliative Care Specialist

About the Role
We are seeking a highly compassionate Registered Nurse to join our virtual team in an Advanced Care Planning and Palliative Care role. Reporting directly to iAgeHealth Manager, you will be an integral part of supporting our residents and their families to make informed decisions around their future health care needs and desires.
Key Responsibilities
- Provide best practice care and advice to elderly residents
- Provide mentoring and leadership to the broader care team
- Ensure high quality outcomes are always delivered
About You
Essential Skills:
- Current APHRA registration as a Registered Nurse
- Postgraduate education in Palliative Care
- Previous experience in an Advanced Care Planning and Palliative Care role
