Capacity Building Coordinator - Population Health

This role presents an exciting opportunity for a passionate and innovative primary health care professional to take on a specialist capacity development position, focusing on Population Health.

The Capacity Development Specialist will work closely with service providers, partners, and stakeholders to ensure the implementation and delivery of commissioned services and build on their existing capabilities.

Key Responsibilities:

  • Support clinical and non-clinical service providers to develop and sustain their capabilities across the primary care sector;
  • Establish and maintain effective partnerships with service providers;
  • Manage contracts with service providers;
  • Facilitate connections and integration between service providers to influence best practice service delivery;
  • Foster a quality improvement approach to evaluation, assessment, monitoring, and redesign;
  • Participate in broader systems-level partnerships and integration.

Required Skills and Qualifications:

  • Tertiary qualifications in health or human services or management (or other relevant discipline); or at a minimum, experience at a senior level within a health organisation;
  • Demonstrated experience in building and influencing beneficial partnerships and professional relationships with a wide range of stakeholders;
  • Experience in contract management;
  • Expertise in critical analysis of complex issues in service delivery and service development;
  • Understanding of, and ability to practice in a manner consistent with, the principles of Primary Health Care;
  • Working knowledge of capacity building principles;
  • Knowledge of population health principles.
