About the Role
  • Consumer-directed services promote independence, dignity, and wellbeing through tailored care plans and assessments.
  • Coordinating client services, overseeing case loads, and providing direct nursing care when required.
  • Identifying and managing clinical risks with a proactive approach to ensure compliance and quality standards.
  • Offering training and support to team members to enhance their skills and confidence.
Your Key ResponsibilitiesClient-Focused Care:
  • Conducting home assessments and preparing individualised care plans that cater to clients' diverse needs.
  • Providing clinical education and support to clients and their families to empower them in their care journey.
  • Monitoring care quality and implementing nursing interventions as needed to ensure optimal outcomes.
  • Responding to service enquiries and referrals, and building strong relationships with external providers and stakeholders.
What We're Looking For
  • AHPRA Registration as a Registered Nurse is essential for this role.
  • Demonstrated leadership and coordination experience in aged or community care, with a strong understanding of aged care legislation and quality frameworks.
  • Excellent interpersonal, problem-solving, and documentation skills are critical for success in this position.
  • Proficiency in using clinical and scheduling software is also desirable.
