About the Role

We are seeking a skilled and compassionate Community Care Worker to join our team at the Eyre and Far North Local Health Network. As a Community Care Worker, you will play a vital role in supporting individuals who are aged, frail, or disabled, enabling them to maintain their independence and live in their own homes with some assistance.

Key Responsibilities
  • Provide high-quality care and support to consumers, including cleaning, transport, and social support.
  • Work collaboratively with consumers, their families, and other health professionals to develop and implement care plans.
  • Contribute to a consumer-directed care model, empowering consumers to make choices about their care and services.
  • Participate in ongoing education and training to develop skills and knowledge in areas such as aged care, disability, and home and community care.

Requirements

  • Able to provide quality outcomes and maintain consumer independence where possible.
Classification and Qualifications

This position is multi-classed as WHA3/WHA4, and classification will be determined by qualifications and experience. To be eligible at WHA4 level, you must have completed Certificate III or higher in Individual Support (Aged Care, Disability, or Home and Community).

  • Relevant qualifications and experience in aged care, disability, or home and community care.
  • Ability to work collaboratively with consumers, their families, and other health professionals.
  • Flexibility and adaptability in a dynamic and fast-paced environment.
