We are seeking a skilled Care Specialist to join our team in Mawson. This is an exciting opportunity to work with an independent, not for profit organisation providing aged care services.

As a Care Specialist, you will be responsible for case management with our Home Care Package clients and actively involved in service promotion. You will also provide advice on access to community-based aged care to community members and work closely with our team of Support specialists.

About the Role
  • This is a permanent full-time position (38 hours a week) offering a competitive salary of $90,000 per annum, commensurate with experience.
  • You will have the opportunity to use your highly developed interpersonal and communication skills to build strong relationships with clients, their families, and our Support team.
  • Our office is located at Southlands Shopping Centre, Mawson, with free all-day parking available.
Key Responsibilities
  • Deliver high-quality case management services to Home Care Package clients, ensuring their needs are met and exceeded.
  • Provide expert advice on access to community-based aged care services, promoting our organisation's services and building client numbers.
  • Work collaboratively with our Support team to deliver person-centred care, focusing on individualised support plans and outcomes.
  • Develop and maintain relationships with local community groups, agencies, and healthcare providers to enhance our service delivery and promote our organisation's mission.
  • Contribute to the development and implementation of new programs and initiatives, aligning with our organisation's strategic goals.
Required Skills and Qualifications
  • Demonstrated experience in client assessment and case management in a community setting, preferably in aged care.
  • Sound understanding of relevant legislation, including the Single Funder Model.
  • Able to work effectively in a small team environment, demonstrating strong communication and collaboration skills.
  • Knowledge of community resources and professional services, with ability to access and refer clients as needed.
  • Culturally sensitive and able to work with diverse groups, adapting your approach to meet individual needs.
