
Care Coordinator for Substance Abuse Services

This permanent part-time (0.8 FTE, 4 days per week) vacancy is for a Clinical Nurse at our integrated services in Fremantle.
The successful candidate will assess, plan, implement and evaluate the care of those identified as having mental health, drug and alcohol related problems, to assess and manage clinical risk and to participate in the development of Alcohol and Other Drugs Services.
- They will manage clinic-based alcohol and other drugs assessment, consultation liaison for withdrawal management, delivery of opioid treatment program, therapeutic interventions and harm minimisation programs.
We are seeking an experienced Clinical Nurse who can work at other Integrated Clinical Service sites based on operational needs.
Key Responsibilities:
- Assess and manage clinical risk
- Develop and implement individualised care plans
- Collaborate with multidisciplinary teams
- Provide education and support to clients
Required Skills and Qualifications:
- Clinical nursing experience in AOD or Mental Health settings
- Ability to understand and act within key boundaries
- Flexible and adaptable approach
- Experience working with complex presentations
