Nurse Care Coordinator

We are seeking a highly skilled and compassionate Nurse Care Coordinator to join our healthcare team. As a Nurse Care Coordinator, you will play a vital role in coordinating and leading the delivery of client-focused care for our residents and clients in the community.

About the Role:

The ideal candidate will possess great interpersonal and communication skills, with the ability to work well in a team environment and independently. You will be responsible for assessing the total health needs of clients, developing individual care plans in consultation with clients and significant others, and ensuring compliance with all relevant legislation and policies.

Responsibilities:
  • Care Coordination: Supervise and direct the delivery of client-focused care delivered by staff, ensuring high-quality care standards are met.
  • Health Needs Assessment: Assess the total health needs of clients, documenting results thoroughly and identifying areas for improvement.
  • Care Planning: Develop individual care plans in consultation with clients and significant others, taking into account their unique needs and preferences.
  • Legislative Compliance: Ensure compliance with all relevant legislation and policies, maintaining accurate records and reporting as required.
Requirements:
  • A valid AHPRA registration
  • An understanding of Commonwealth Aged Care standards & Quality of Care principles
  • Good knowledge of AN-ACC requirements
  • Proven ability to multitask and work under pressure
  • Great interpersonal and communication skills
