Key Responsibilities:- Provide personalized support to clients with various tasks such as personal care, household chores, transportation, and community engagement.
- Assist clients in achieving their personal goals and aspirations.
- Work collaboratively with clients, their families, and other healthcare professionals to develop and implement individualized care plans.
- Conduct regular assessments to identify client needs and develop strategies to address them.
- Maintain accurate records and reports of client interactions and progress.
- Experience in disability support work, aged care, or a similar field.
- Good communication skills and the ability to build relationships with clients and their families.
- Empathy, compassion, and a genuine interest in improving the lives of others.
- Ability to work independently and manage your time effectively.
- Flexibility to work different shifts, including weekends and evenings.
- First Aid and CPR certifications, or a willingness to obtain them.
- A valid driver's license and reliable transportation.
- Bonus points for holding a Cert III in Individual Support, Aged Care, Disability Care, or similar qualifications.
- Worker screening clearances, or the ability to obtain one (NDIS Worker Check)
