About the Role

We are seeking a highly skilled CDM Nurse to join our team at ForHealth Medical Centres. As a Chronic Disease Management Nurse, you will play a vital role in supporting our patients to manage their chronic conditions and improve their overall health.

Key Responsibilities
  • Develop and implement care plans to help patients manage their chronic diseases
  • Empower patients to take ownership of their health and well-being
  • Conduct health assessments to identify potential health risks and develop strategies to mitigate them
  • Work closely with GPs and other healthcare professionals to provide comprehensive care to patients
  • Monitor patient progress and adjust care plans as needed
Requirements
  • AHPRA registered Enrolled or Registered Nurse
  • Proven experience in Chronic Disease Management and health assessments
  • Strong organisational and time management skills
  • Excellent communication and customer service skills
  • A passion for delivering high-quality patient care
