Care Coordinator for Elderly and Disabled Individuals

Role OverviewWe are seeking a highly skilled and compassionate Personal Care Worker to join our team. As a key member of our home care team, you will provide assistance to our clients, ensuring they receive the highest standard of care and support in their own homes.
Key Responsibilities- Provide personal care and assistance to clients, including help with daily living tasks, transportation to appointments, and social activities
- Develop and implement personalized care plans to meet the unique needs and preferences of each client
- Work collaboratively with other healthcare professionals to ensure seamless delivery of care
To be successful in this role, you will have:
- A valid working visa for Australia
- A police check (valid within the last 3 years)
- Flu vaccinations or willingness to obtain
- An owned vehicle and license
- Evidence of COVID-19 vaccinations
- Experience in a care role, although not essential
