Clinical Care Coordinator & Nurse Mentor Role

This is an exciting opportunity to work in a dynamic care environment as a Clinical Care Coordinator and Nurse Mentor.

The Job Description:
  • Provide expert clinical leadership and support to our care teams.
  • Collaborate with carer teams to meet all clinical needs of residents, including monitoring and tracking clinical indicators.
  • Manage assessments, documentation, and submission of accreditation requirements within the facility.
  • Ensure effective documentation and full team user support of policies, procedures, and IT system requirements.
Responsibilities:
  • Coordinate assessment, care planning, and review processes within the facility.
  • Support the admission of new residents for optimal care assessments and initial consultations.
  • Ensure accurate information is recorded and compliant with organisational and legislative requirements.
  • Input and oversight into facility team rosters and ensure optimal skill mix for resident care delivery.
  • Manage and document clinical risk within the facility and drive continuous improvements.
Qualifications and Requirements:
  • Minimum 1 year experience in a similar leadership position or as a Care Coordinator.
  • Current unrestricted Registered Nurse Div 1 registration with the Australian Health Practitioners Regulation Agency.
  • Post graduate qualifications in aged care are advantageous but not essential.
  • Extensive knowledge of aged care clinical assessments, care plans, AN-ACC compliance and documentation.

Our ideal candidate will possess excellent communication and interpersonal skills, be able to manage multiple stakeholders, and have high computing skills for maintaining documentation and reporting.
