Job Overview

This is a highly rewarding position, assisting individuals to readjust to living within their communities after long periods of admission in mental health inpatient units or similar institutions.

Your key responsibilities include:

  • Providing ongoing recovery orientated care coordination with consumers;
  • Supporting and empowering consumers to undertake daily living skills, budgeting and medication management;
  • Working alongside consumers to create individualised support plans that are in line with consumer's personal goals and needs;
  • Creating and updating individual consumer files, notes and reports as required;
  • Developing and maintaining relationships with internal and external stakeholders including family/supports of consumers, service partners and government agencies;
  • Contributing to the effective functioning and development of our services through involvement in projects, contribution to team forums and training and development;

Requirements for success include:

  • A relevant tertiary qualification, such as social work, welfare or psychology; persons nearing the end of their course are also encouraged to apply;
  • Understanding of the recovery orientated principles and framework, as well as psycho-social rehabilitation;
  • A valid driver's license.
