Job Overview:

The Care Management Specialist is responsible for overseeing patient care and ensuring seamless transitions across the healthcare continuum.

Key Responsibilities:
  • Manage waiting list forms from specialist clinics, private rooms, and other areas of SVHM within designated timeframes.
  • Create, record, and update patient information on the Patient Administration System (PAS) accurately and efficiently.
  • Forward patient queries to Surgical Liaison Nurses for clinical management.
  • Maintain Category 1 patients' admission timelines within 30 days.
  • Monitor Category 2 and 3 patients' access to surgery within designated timeframes.
Requirements:

This role requires strong organizational skills, attention to detail, and effective communication. The successful candidate will possess a high level of proficiency in managing multiple priorities and deadlines.

What We Offer:

HS1 - HS17 ($58,442 - $64,610 per annum pro rata) + over award ($4.4447 per hour)
