Job Opportunity

We are seeking a skilled and compassionate Clinical Assessor to join our team in Hervey Bay. The successful candidate will conduct comprehensive assessments using the Integrated Assessment Tool (IAT) to evaluate customer needs.

Key Responsibilities:
  • Conduct thorough evaluations of customer requirements, utilizing the IAT to ensure accurate assessments.
  • Create tailored support plans and facilitate connections with relevant service providers to meet customer needs.
  • Collaborate effectively with customers, carers, families, and other stakeholders in the aged care planning process, ensuring seamless communication and mutual understanding.
  • Manage changing customer and carer needs, guaranteeing that service recommendations align with Delegate decisions.
  • Identify customers' formal and/or informal respite support arrangements and develop Emergency Care Plans as needed.
Requirements:
  • Tertiary qualification in a health-related discipline, such as Registered Nurse, Physiotherapist, or Social Worker.
  • Current unrestricted registration with AHPRA or eligibility for a relevant professional association.
  • Minimum one year experience delivering services in aged care settings or to aged persons.
  • Knowledge or experience in conducting home assessments or risk assessments for vulnerable individuals.
  • Ability to communicate effectively with customers who have sensory loss and/or cognitive impairment.
  • Flexibility and adaptability to respond to changing task requirements.
