Home Care Provider

We are seeking an enthusiastic and kind person to provide quality care to clients in their own home.
Our ideal candidate will deliver personal care and support with daily living activities, work collaboratively with our team to achieve the best outcomes for individuals, and provide social outings and transport services.
The successful candidate will also provide in-home respite guided by a Care Plan, monitor and report on changes to client's health through appropriate protocols, and build strong relationships with clients and their families.
Responsibilities:
- Delivering personal care and support with daily living activities
- Collaborating with our team to achieve the best outcomes for individuals
- Providing social outings and transport services
- Providing in-home respite guided by a Care Plan
- Monitoring and reporting on changes to client's health through appropriate protocols
- Building strong relationships with clients and their families
Requirements:
- Driver's license and comprehensive car insurance (essential)
- Certificate III in Individual Support / Certificate III Aged Care or similar qualification (preferred)
- Prior experience in Aged Care / Disability capacity (preferred)
- Excellent communication skills (verbal & written)
- Compassion / Empathy
