This role offers a unique opportunity to drive meaningful impact in the community by integrating aftercare services across South Western Sydney Local Health District. You will work closely with stakeholders to embed and strengthen Aftercare pathways, ensuring consistent, high-quality care for individuals following a suicidal crisis.
Main Responsibilities:
- Coordinating stakeholder engagement and service integration to enhance referral pathways and service improvements.
- Supporting data reporting, evaluation, and quality initiatives to ensure best practice approaches are embedded across the district.
- Promoting awareness and understanding of Aftercare services among staff, patients, and the broader community.
To excel in this position, you must possess strong leadership skills, excellent communication abilities, and a deep understanding of mental health and suicide prevention principles. Collaborative working with diverse stakeholders, including government agencies, health networks, and community service providers, is essential.
About the Role:
The vision for South Western Sydney Local Health District is Leading Care, Healthier Communities. As a Lead Integration Specialist, you will play a critical role in achieving this vision by providing district-wide leadership for the integration of aftercare services. This includes coordinating with hospital and community mental health teams to ensure access to aftercare support is available for all who need it.
