We are seeking an experienced Social Worker to join our client based in Canberra, working on a permanent full-time basis across a range of hospital and community settings.
Key Responsibilities- Provide high-level clinical assessment and psychosocial interventions across various clinical areas, including individual and group work with consumers and carers.
- Exercise independent professional judgment in solving problems and managing cases, adapting principles, procedures, and techniques as needed.
- Liaise with patients, carers, and service providers to enhance and improve clinical outcomes and develop partnerships for health promotion and other service activities.
- Contribute to the supervision and education of staff and students, participating in supervision, continuing professional development, and performance management.
- Actively participate in team meetings, complete clinical and administrative data collection, and maintain case records to a consistent high standard.
- Practice in accordance with professional body codes of conduct, practice standards, and organisational guidelines and legislative requirements.
- Proven knowledge of skilled clinical psychosocial assessment, counselling, and complex interventions, responsive to multi-faceted patient needs.
- Demonstrated experience working collaboratively within a multidisciplinary team, with strong communication skills and ability to establish effective working relationships.
- Critical self-reflection and active participation in professional supervision, with experience in undertaking innovative quality improvement activities.
- Understanding of safety and quality standards, work, health and safety (WH&S), and the positive patient experience.
- Displays behaviour consistent with CHS's values of reliable, progressive, respectful, and kind.
- Relevant Social Work degree/qualifications and a minimum of 3 years' experience working professionally in hospital or health is preferred.
- Be registered (or eligible for registration) with the Australian Association of Social Workers (AASW).
Annual remuneration package: $105,900 – $119,774 (Plus 11.5% Super)
