Role Summary

This is a 6 month contract position that supports the Clinical Funding Resource Manager in optimising clinical funding claims through monitoring of clinical assessments. The Admissions & Reassessments Consultant will work with internal and external stakeholders to ensure congruency between the care provided to residents, resident assessments, care plans and the funding received.

The consultant will be responsible for supporting the Care Homes to achieve funding outcomes by taking accountability for identifying and submitting appeals against Australian National Aged Care Classification (AN-ACC) funding that is in dispute.

The role requires strong knowledge of the AN-ACC system, as well as excellent communication and mentoring skills.

Responsibilities
  • Support the home with effective completion of resident documentation and charting.
  • Engage with and monitor resident's and their care needs with the view towards instilling effective processes within the home to identify and manage changes in care needs.
  • Audit and evaluate assessments prior to the external AN-ACC assessors visit to ensure congruency and accuracy.
  • Mentor and coach care Home Leadership Team in effectively managing their assessments on admission in preparation for external AN-ACC assessors visit.
  • Develop and maintain open and regular communications and relationships with all members of staff.
  • Work in conjunction with the Clinical Funding Resource Manager on the submission and monitoring of requests for reassessments.
  • Provide training and where feasible manage the training needs of the home in relation to documentation and assessments.
  • Liaise with external AN-ACC assessors, supports the care home to prepare for an AN-ACC assessment.
Requirements
  • Knowledge of the Australian National Aged Care Classification (AN-ACC).
  • Tertiary qualifications in training and assessment desirable.
  • Registration with the Australian Health Practitioner Regulation Agency desirable.
  • Experience in clinical care management with sound knowledge and experience.
  • Ability to provide regular ongoing support directly within our Homes on a regular basis.
  • Mentoring and coaching skills.
  • Experience in liaising with key regulatory bodies.
  • Change management experience.
  • Proven written and communication skills.
  • Advanced Clinical Assessment Skills.
