About the Role

We are seeking a passionate and experienced Grade 2 Occupational Therapist to join our Community Therapy team at Northern Health. As an Occupational Therapist, you will provide high-quality services to patients/participants within the Commonwealth Home Support Program (CHSP), National Disability Insurance Scheme (NDIS), and Home and Community Care (HACC) programs.

Key Responsibilities
  • Provide occupational therapy services to patients/participants within the CHSP, NDIS, and HACC programs.
  • Work collaboratively with the Community Therapy team to achieve positive patient outcomes.
  • Develop and implement individualized treatment plans to support patients/participants in maintaining their independence and preventing premature or inappropriate admission to long-term residential care.
  • Provide education and support to patients/participants and their families/caregivers to promote healthy living and community participation.
Requirements
  • Experience working with clients living within the community.
  • Proven ability to work well in a team environment.
  • Interest in working with older adults (65+ years).
  • A positive attitude to lifelong learning.
