As a senior case manager, you will play a pivotal role in developing and implementing care plans that cater to the unique needs of children and young people.

Key responsibilities include engaging families, carers, and other stakeholders in case plan formulation and implementation. This involves conducting assessments, planning, reviews, and referrals for children and young people with complex support needs.

You will work closely with internal and external stakeholders to improve service delivery and achieve care goals. This may involve collaborating with colleagues to develop positive relationships with children and young people, applying the CARE model principles in all interactions.

As a leader in best practice assessment, planning, and evaluation processes, you will provide regular supervision, coaching, development, and support to case management staff. This will enable them to deliver quality outcomes and contribute to the organisation's mission to create safe, supportive environments for vulnerable populations.

To be successful in this role, you will possess excellent written communication skills, enabling you to create reports, correspondence, and other materials. A recognised degree qualification in Social Work, Social Sciences, Psychology, Nursing or Mental Health is desirable but not essential for Aboriginal and/or Torres Strait Islander applicants with relevant experience.
