Aged Care Telehealth Nurse Practitioner

We are seeking a skilled Advanced Practice Registered Nurse (APRN) with expertise in aged care telehealth to join our team. This flexible locum role allows you to work from the comfort of your own home while making a meaningful impact in the lives of elderly individuals.
Key Responsibilities:- Provide comprehensive nursing assessment, diagnosis, intervention, and management to aged care residents via secure video conferencing platforms.
- Address acute health issues, update care plans, and coordinate care with facility staff, primary care physicians, and specialists as needed.
- Issue electronic prescriptions, order/review investigations, and make referrals to telehealth specialists when required.
- Conduct regular virtual rounds and ensure timely documentation to support continuity of care.
- Contribute to the ongoing development and improvement of our innovative after-hours model of care.
- Current registration as an APRN with a valid license to practice in their state/province.
- Advanced clinical experience in assessment, diagnosis, and treatment relevant to aged care.
- Strong decision-making, communication, and collaboration skills.
- Proficiency with telehealth systems, electronic medical records, and other healthcare technologies.
- Professional liability insurance and a reliable laptop or desktop computer.
