About This Role:

This position involves delivering clinical services, including care plan assessments, medication management, wound care, chronic disease management, palliative care, and diabetes management in a home setting.

You will oversee non-clinical team members and coordinate intake processing, care planning, and reporting in a community environment.

Conducting client care assessments focusing on Consumer Directed Care is another key responsibility.

Coordinate person-centred care according to client needs, service/care plans, and available funding while ensuring compliance with AHPRA regulations and legislation.

General administration tasks related to client documentation and reporting are also essential.

A flexible on-call roster is part of the role.

Required Qualifications and Skills
  • Australian National Police check
  • Professional Indemnity Insurance
  • Experience in assessing and planning care needs within the community
  • A valid Driver's License and reliable vehicle
  • A smart mobile phone (iPhone/android)
Preferred Skills
  • A passion for high-quality, client-focused care in a community setting
  • Cert IV in Training and Assessment
  • Ability to work autonomously or within a team environment
  • Excellent time management and organisational skills
  • Ongoing professional development commitment
