Role Overview: This position is designed to provide intensive clinical support to individuals at risk of frequent mental health relapses.
The role aims to reduce emergency department presentations and readmissions by delivering timely and effective care. Strong communication and interpersonal skills are essential for success in this position.
Key responsibilities include working collaboratively with other community clinicians and providing high-quality support to a targeted consumer cohort.
Mandatory requirements include proof of qualification and registration with the relevant Australian registration authority or association.
