Job Summary:

We are seeking an experienced Registered Psychiatric Nurse to join our Merri-bek Community Team in Coburg as a Senior Physical Health Nurse. The successful candidate will be responsible for overseeing the local Physical Health Program, supporting the team in implementing services objectives, and providing metabolic monitoring and physical health assessments and interventions.

Key Responsibilities:

  • Develop and implement processes and guidelines to improve physical health outcomes for consumers with complex psychiatric needs.
  • Work closely with General Practitioners and other primary healthcare providers to build partnerships and ensure appropriate physical health follow-up for consumers.
  • Identify key gaps in provision of physical health assessment and intervention for the team and develop strategies to address these.
  • Provide leadership and support to the team in implementing the Physical Health Program.
  • Collaborate with other clinicians to develop and implement physical health interventions and assessments.

Requirements:

  • Appropriate qualifications and registration with AHPRA.
  • At least 5 years' experience post-qualification.
  • Postgraduate qualification in mental health.
  • Understanding of the Equally Well Framework and its application to community mental health teams.
  • Proven leadership skills and ability to work autonomously.
