The Role

As a Support Worker, you will be responsible for delivering personalized services to clients in their homes and communities, as outlined in their Personal Independence Plan.

  • Deliver care and support services to clients, adhering to their individual needs and goals.
  • Contribute to assessments of clients' interests, needs, and goals, ensuring effective communication with carers and family members.
  • Maintain accurate records and case notes, adhering to risk mitigation procedures and reporting new risks to the service coordinator.
  • Undertake administrative duties associated with the role, including mileage tracking and vehicle maintenance.

This casual opportunity is available for a minimum of 15 hours per week, Monday to Friday.

About You

  • Effective interpersonal and communication skills, maintaining confidentiality and impartiality at all times.
  • Respect and responsiveness to client needs, working collaboratively with colleagues and other stakeholders.
  • A reliable vehicle with full comprehensive insurance, as well as a willingness to adapt to non-traditional approaches to service delivery.
  • Fully vaccinated for COVID-19 and maintained a NDIS Worker Screening check.
