**Job Title:** Clinical Care ProfessionalWe are seeking a compassionate and skilled individual to join our care team as a Registered Nurse or Enrolled Nurse. This role involves providing high-quality, personalised care to clients in their homes, supporting them to maintain their independence and quality of life.About the Role

As a Registered Nurse or Enrolled Nurse, you will be responsible for delivering care following the client’s care plan, meeting their safety needs, and conducting nursing assessments.

Key Responsibilities:
  • Deliver person-centred care to clients in their homes, ensuring their physical, emotional, and social needs are met
  • Develop and implement individualised care plans to support clients in achieving their goals and outcomes
  • Conduct regular nursing assessments to identify any changes in a client's condition and adjust their care plan accordingly
  • Maintain accurate and up-to-date records and documentation
Requirements:
  • AHPRA Registration (Australian Health Practitioner Regulation Agency)
  • Registered or Enrolled Nurse qualification
  • CPR & First Aid Certification
  • Car and valid driver's license
  • National Police Check (Aged Care/Vulnerable Persons Sector)
  • Working With Children Check
  • NDIS Worker Screening Approval
  • NDIS Worker Orientation Module Completion
