Overview

The role of a Triage and Connect Complex Case Manager is pivotal in addressing the needs of clients with complex mental health conditions.

This position requires an individual who can provide assessment, case management, and clinical expertise to support and enhance the work of other team members.

The ideal candidate will have a strong background in a relevant health field, including nursing, psychology, occupational therapy, or social work.
