Our team is dedicated to providing exceptional healthcare services in a collaborative and supportive environment.
We are seeking a highly motivated Comprehensive Health Solutions Specialist with a passion for delivering behaviour change therapy to work with clients managing chronic conditions such as diabetes, respiratory disease, and heart disease.
About the Role:
The Comprehensive Health Solutions Service comprises of interdisciplinary teams working together to deliver comprehensive care to clients. Our team works collaboratively and in alignment with other services within the network as well as local Aboriginal Health Services, with a focus on optimising functional and health outcomes and reducing hospital admissions for clients with Diabetes, Chronic Cardiac and Respiratory Conditions and vulnerable people with other chronic conditions.
Responsibilities:
- Collaborate with clients to set realistic goals and care plans to achieve lasting lifestyle changes
- Provide ongoing motivation, education and support to encourage self-management strategies
- Work within an interdisciplinary team of healthcare professionals to deliver comprehensive care
- Continuously evaluate and adjust therapy techniques to ensure the best possible outcomes for each client
Requirements:
- Possess knowledge and experience working with clients with chronic conditions and an understanding of the daily impact on clients lives (particularly diabetes, respiratory disease and heart disease)
- Have a passion for helping others achieve their health and wellness goals
- Possess strong communication skills and the ability to build trust and rapport with clients
- Have desirable experience in behaviour change, health coaching or chronic disease management and experience with evidence-based behaviour change frameworks (eg motivational interviewing, CBT etc)
- Have the ability to work independently as well as part of a multidisciplinary team
- Show empathy and patience with a client-centred approach to care
Eligibility:
- Hold an appropriate degree or equivalent qualification for one of the following Allied Health disciplines working in the service, specifically:
- - Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Dietitians Association of Australia (DAA).
- - Appropriate Degree or equivalent qualification, usually in human movements, inclusive of the exercise physiology elective and eligible for accreditation from Exercise Sports Science Australia (ESSA).
- - Appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia.
- - Appropriate Degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia.
- - Appropriate Degree or equivalent qualification which entitles registration with the Podiatry Board of Australia.
- - Appropriate Degree or equivalent qualification recognised by the PsyBA and general registration with PsyBA.
- - Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
A certificate in the Flinders Model of Chronic Condition Management (or willingness to achieve this with in-house training) is highly desirable.
