Complex Needs Support Specialist

We are seeking a highly skilled and experienced professional to join our team as a Complex Needs Support Specialist. In this role, you will be responsible for working closely with individuals who require support to achieve their goals.

About the Role:

  • Provide individualized support to complex NDIS participants to help them achieve their goals and outcomes.
  • Work closely with participants, their families, and other stakeholders to develop and implement personalized support plans.
  • Conduct needs assessments and identify strategies to support participants in achieving their desired outcomes.
  • Maintain accurate records of participant interactions, progress, and outcomes.
  • Collaborate with other professionals, including healthcare providers and social workers, to ensure comprehensive support services are provided.
  • Demonstrate a strong understanding of the NDIS, its policies, and procedures.

Requirements:

  • A Diploma of Community Services or related field.
  • Previous experience as a Specialist Support Coordinator or similar role.
  • Demonstrated ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Able to maintain confidentiality and handle sensitive information discreetly.
  • Possess a valid driver's license and willingness to travel (75% of the time).
