Care Professional Opportunity

Our team is seeking a full-time care professional to join us in delivering person-focused home care services.

This role plays a critical part in our efforts to make a meaningful difference in the lives of older Australians every single day. We are looking for someone who can conduct client assessments, develop individualised care plans, coordinate and monitor service delivery, document and maintain accurate client records, collaborate with teams, advocate for client wellbeing, and identify additional support needs.

Key Responsibilities
  • Client Assessments: Conduct thorough assessments of clients to determine their needs and develop tailored care plans.
  • Service Delivery: Coordinate and monitor the delivery of home care services to ensure quality and compliance.
  • Record Keeping: Maintain accurate and up-to-date records of client interactions and care plans.
  • Team Collaboration: Work collaboratively with other teams to ensure smooth service delivery.
  • Client Advocacy: Advocate for the wellbeing and dignity of clients, while respecting their confidentiality.
  • Additional Support: Identify additional support needs and facilitate referrals where necessary.
Requirements
  • A Certificate IV in Ageing Support or an Enrolled Nurse qualification is required for this role.
  • You will also need to have current First Aid and CPR certification, an unrestricted driver's licence, and own insured, registered vehicle.
  • Strong interpersonal and communication skills are essential for success in this position.
