Delivering Exceptional Home Care Services

We are seeking a dedicated and compassionate professional to join our team of healthcare providers as a Home Care Partner. In this rewarding role, you will have the opportunity to make a positive impact on the lives of older Australians by delivering high-quality, person-focused home care services.

Your key responsibilities will include conducting comprehensive client assessments, developing individualized care plans, and coordinating and monitoring service delivery to ensure quality and compliance. You will also be responsible for documenting accurate client records, collaborating with internal teams, and advocating for client wellbeing while respecting confidentiality and dignity.

To succeed in this role, you will require excellent communication and interpersonal skills, as well as the ability to work independently and collaboratively within a team. A Certificate IV in Ageing Support or Enrolled Nurse qualification is essential, along with current First Aid and CPR certification and an unrestricted driver's license.

Key Skills and Qualifications:
  • Certificate IV in Ageing Support or Enrolled Nurse (Diploma of Nursing)
  • Current First Aid and CPR certification
  • Unrestricted driver's license and own insured, registered vehicle
  • Excellent communication and interpersonal skills
  • Able to work independently and collaboratively within a team
