**The Role**
We are seeking an experienced Client Care Coordinator to join our team. The successful candidate will be responsible for directing and coordinating all activities associated with the care of our NDIS clients, working closely with our Care Coordination Support team.
**Responsibilities**
- Developing and implementing care plans that meet the unique needs and goals of our clients.
- Coordinating with our office team to ensure seamless delivery of care services.
- Managing the overall client experience, ensuring that their needs and preferences are met.
- Providing specialist support to adults with chronic and degenerative conditions.
**Requirements**
- Experience in the NDIS Industry and understanding of the NDIS frameworks.
- Excellent organisational skills, with the ability to prioritise and manage multiple tasks.
- A strong focus on customer service, with a commitment to delivering exceptional care and support.
- A team player with a positive attitude and a passion for delivering high-quality care.
- Competent in using computers and systems, with experience in administrative roles.
