Position Overview

Healthe Care Group is seeking a highly skilled and compassionate Patient Assessor to join our team.

In this critical role, you'll be responsible for conducting comprehensive pre-assessments of referred patients for medical/rehabilitation, general medical, and mental health admissions.

As a Regional Patient Assessor, you'll work closely with multidisciplinary teams to evaluate potential admissions, undertake clinical risk assessments, and ensure appropriate health funding supports the admission of patients for speciality services.

  • Conducting detailed patient assessments to evaluate potential admissions for rehab, medical or mental health patients
  • Undertaking clinical risk assessment of all potential hospital admissions to determine prioritisation and suitability
  • Ensuring appropriate health funding supports the admission of patients for speciality services
Accountabilities
  • Acting as a conduit between the hospital and VMOs in relation to potential admissions
  • Providing accurate and timely information to Nurse Unit Managers on expected admissions
  • Liaising with our accounts department to ensure all patients are informed of out-of-pocket expenses prior to being accepted for admission
