We are seeking an experienced Social Worker to join our dynamic Allied Health team at Albury Wodonga Health. This role is part of our interdisciplinary team based in the hospital, working closely with medical, nursing, and other allied health professionals to provide high-quality Social Work service provision.
Key Responsibilities:
- Provide high-quality Social Work service provision to patients and their families
- Work collaboratively with the clinical team to optimize patient flow and facilitate suitable care pathways
- Develop and implement individualized care plans to meet the needs of patients
- Contribute to the development and implementation of policies and procedures related to Social Work practice
Requirements:
- Relevant qualifications and experience in Social Work
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Flexibility to work a variety of shifts, including weekends
