As a key member of our multidisciplinary team, you will provide high-quality assessment, intensive case management services, and assertive outreach support to individuals experiencing mental illness.
Main Responsibilities- Clinical Case Management: Work with individuals with a lived experience of mental illness to develop and implement personalized recovery plans.
- Assertive Outreach Support: Provide intensive support and guidance to individuals in their homes, communities, or other settings to promote recovery and well-being.
