About the Role

This is an exciting opportunity for a dedicated professional to work with our multidisciplinary team and make a positive impact on the lives of Aboriginal children and their families.

 

The Aboriginal Health Worker will be responsible for providing culturally responsive assessment and care coordination services for Aboriginal children who are in Out of Home Care. This role requires strong communication and interpersonal skills, as well as the ability to engage with clients and carers, identify and consider cultural needs as part of assessment, support clinicians to provide culturally responsive care and facilitate care coordination as part of the multidisciplinary team.

 

Key responsibilities include:

  • Providing culturally responsive assessment and care coordination services for Aboriginal children in Out of Home Care
  • Engaging with clients and carers to understand their needs and develop effective care plans
  • Working closely with a variety of professionals, including clinicians, social workers, and other healthcare professionals, to ensure comprehensive care and support
  • Facilitating care coordination and supporting the development of health management plans
  • Conducting regular reviews and assessments to ensure that care plans are meeting the needs of Aboriginal children and their families
