About the Role

The Homeless Health Outreach Team provides support to individuals experiencing homelessness or at risk of homelessness within a designated region. The service employs an assertive outreach model, delivering comprehensive assessments, care coordination, and clinical interventions tailored to address mental health and substance use challenges.

Key Responsibilities
  • To provide expert advice and assistance in assessing and treating individuals experiencing mental health and substance use challenges
  • To collaborate with multidisciplinary teams to coordinate care and ensure continuity of service delivery
  • To develop and maintain relationships with community partners to enhance access to services and support for individuals experiencing mental health and substance use challenges
Requirements
  • Mandatory requirements include relevant tertiary education and appropriate professional registration. Please refer to the role description for further information regarding qualifications and membership.
  • A Class Motor Vehicle Licence is required.
  • Pre-employment screening will be conducted for the successful applicant, including Criminal History checks.
  • This role is classified as VPD risk and may require evidence of vaccination or immunity to specified diseases.
  • A probationary period may apply.
