About the Role:
- This is an exciting opportunity for a dedicated and experienced Nurse Practitioner to join our team in Hunter Valley, New South Wales.
- You will work across three residential care homes, each with 50 beds, providing high-quality care to elderly residents.
- Your responsibilities will include collaborating with multidisciplinary teams, leading advanced care planning discussions, and reviewing new treatments and technologies.
- You will also provide educational resources for residents, families, and carers, and participate in ongoing education and mentorship activities.
Key Responsibilities:
- Clinical Leadership: Review new treatments, technologies, and therapeutic interventions to ensure the highest standards of care.
- Patient Education: Provide educational resources for residents, families, and carers, supporting informed decision-making and empowering individuals in their care.
- Record Keeping: Maintain clear, concise, and accurate resident records, ensuring compliance with healthcare standards.
- Continuous Professional Development: Participate in ongoing education and mentorship activities to further develop your clinical expertise.
Requirements:
- Qualifications: Registered Nurse with postgraduate qualifications as a Nurse Practitioner.
- APHRA Endorsement: Must hold or be eligible to apply for endorsement with the Australian Health Practitioner Regulation Agency (APHRA).
- Experience: A minimum of 5 years as a Registered Nurse, with at least 3 years of full-time experience in aged or community care settings.
- Clinical Expertise: Strong knowledge of the Nurse Practitioner Standards of Practice as set by the Nursing and Midwifery Board of Australia.
- Patient-Focused Approach: A passion for patient care and a collaborative, team-oriented mindset.
- Excellent Communication: Strong interpersonal skills and the ability to engage with patients, families, and healthcare teams compassionately and effectively.
