Community Mental Health Professional

We are seeking a dedicated individual to join our multidisciplinary team in Perth. As a key member of our Community Treatment Team (CTT), you will work collaboratively with experienced professionals to deliver recovery-focused, person-centred care to consumers of the Community Mental Health Service.

  • Key Responsibilities:
  • Act as a Care Coordinator, managing an allocated caseload and conducting clinical assessments for people with mental health disorders.
  • Plan, implement, and coordinate care to promote consumer safety, quality, and independence.

  • This is an entry-level position ideal for recent graduates or those looking to gain experience in community mental health.

Requirements:

  • Qualified in Social work (or due to graduate soon).
  • Demonstrated ability to meet the Essential Skills Criteria outlined in the Job Description Form (JDF).
