Job Title: Family Wellbeing Service Practitioner

We are seeking a skilled and compassionate Family Wellbeing Service Practitioner to join our team. As a key member of our organization, you will work with families in partnership to empower and build their agency to make effective change and overcome identified family challenges.

Your primary role will involve working closely with clients to identify their goals and develop personalized care plans. You will also be responsible for delivering targeted interventions and facilitating group meetings to support client achievement of their objectives.

About the Role

The successful candidate will have a strong understanding of social services principles and practices, as well as excellent communication and interpersonal skills. You will be able to work effectively in a performance-driven environment, meeting individual weekly, monthly, and quarterly performance targets.

Key Responsibilities
  • Meet individual case performance targets as per weekly, monthly and quarterly dashboard system.
  • Provide high-quality and intent-based practice in all interactions with clients, consistent with ATSICHS values and vision.
  • Provide high-quality case management practices and record keeping that adhere to operational requirements.
  • Foster a client-led assessment and Care Plan to identify the client's goals, including the entire family, where appropriate.
  • Deliver targeted interventions and family facilitated group meetings in achieving client goals.
  • Commit to a high level of quality management for ATSICHS through use of LogiQC quality management systems and MobNet tasks.
Required Skills and Qualifications
  • Current Bachelor's Degree in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention, or Criminology with 2 years' relevant experience in social services field.
  • Or Current Diploma in Community Services, Social Work, Human Services, Child and Family, Allied Health, Youth Work, Child and Family Intervention with 3 years' experience in social services field and at least 1 year of abovementioned experience in a case management role.
  • Ability to work in a performance-driven environment to meet individual weekly, monthly, and quarterly performance targets.
  • Capacity to build client agency, self-efficacy, and facilitate change empowerment, while discouraging client dependence on supports.
  • Knowledge and ability to provide behaviour change interventions that build client self-awareness and active change in achieving their identified goals.
  • Ability to work with the whole-of-family, irrespective of primary client, in meeting the totality of family needs.
  • Experience with case management for high-risk and/or crisis clients, with high-level planning and organisation skills.
