Job Overview

This position offers a unique opportunity for a Social Worker to deliver high-quality clinical services in a Residential Transition Care environment. The role involves working with a variety of caseloads and requires the ability to make sound judgments and decisions under supervision.

About the Role

The successful candidate will be responsible for providing case management services, including liaison with patients and their families, other health professionals, supervisors, students, and researchers. They will also be required to actively participate in the multidisciplinary team's clinical environment to ensure that social work services are integrated, efficient, and equitable for the community served.

Key Responsibilities

Requirements

To be successful in this role, you will require:
