- Client Care Assessments: Conduct thorough client care assessments to identify needs and develop personalized care plans.
- Staff Supervision and Management: Supervise and manage care staff to ensure high-quality care delivery.
- Scheduling and Coordination: Schedule appointments and in-home services with care staff based on discipline, availability, and geographical requirements.
- Care Coordination: Coordinate the provision of quality person-centered care according to client needs, service/care plans, and available funding.
- Clinical Services: Provide clinical in-home services, including care plan assessment, medication management, incontinence care, chronic disease management, complex wound management, palliative care, and diabetes management.
- General Administration: Perform general administrative tasks related to care delivery.
- Qualifications: Qualified Registered/EEN Nurse with current AHPRA registration.
- Australian National Police Check: Must have a valid Australian National Police check.
- Professional Indemnity Insurance: Must have professional indemnity insurance.
- Experience: Experienced in assessing and planning care needs within the community.
- Driver's Licence: Must have a valid driver's licence and a reliable, presentable vehicle.
- Client-Focused Care: Passion for high-quality client-focused care in a community setting.
- Communication Skills: Exceptional communication skills.
- Autonomy and Teamwork: Able to work autonomously or within a team environment.
- Time Management and Organisation: Excellent time management and organisational skills.
