Working as an Aboriginal Health Worker in palliative care requires strong cultural leadership and advocacy skills to facilitate culturally tailored care for Aboriginal people, their families and community facing life limiting illnesses.
You will work collaboratively with the Social Work and Palliative Care Department to lead better community and stakeholder engagement, awareness and education.
Whilst this is a Full-time position, part time applicants and job share could be considered.
Duties- Provide culturally sensitive care coordination for patients, families and communities receiving inpatient palliative care.
- Culturally lead and advocate for the development of departmental models of care, including culturally safe bereavement services and referral pathways.
- Work as an expert member of the multidisciplinary team to ensure that individualised assessment, care planning and management are tailored to meet the unique needs of each Aboriginal client, their family and significant others.
