Care Manager

The role of the Care Manager is to ensure a comprehensive client-centered management plan is achieved for our clients.
This involves conducting initial assessments, creating personalized care plans and setting goals, as well as coordinating services to support clients receiving care at home.
Key Responsibilities:
- Conduct initial assessments, create personalized care plans and set goals with clients.
- Coordinate services to support clients receiving care at home.
- Build and maintain effective relationships with various stakeholders, including healthcare professionals and specialists.
- Monitor patient progress and update care plans as necessary.
- Provide support, education and advice to clients and their families.
Our ideal candidate will have 3-5 years of experience in acute hospital settings as a Registered Nurse with unrestricted registration. Recent experience in care management is highly desirable.
The successful applicant will be customer-focused, with strong clinical reasoning skills and the ability to work in a fast-paced environment.
They will also possess proficient computer skills to enable fast and accurate data entry into patient electronic medical records.
