We are seeking an experienced Clinical Nurse Practitioner to join our dynamic team at integratedliving Australia Ltd.

The Clinical Nurse Practitioner will be responsible for assessing, planning, implementing, and evaluating client care plans, providing expert clinical advice, and collaborating with other healthcare professionals to ensure seamless care delivery. What You Will Be Doing- Providing comprehensive assessments and developing individualized care plans that address the physical, emotional, and social needs of clients.
- Delivering evidence-based interventions and treatments to promote health outcomes and prevent complications.
- Collaborating with multidisciplinary teams to ensure continuity of care and shared decision-making.
- Providing education and support to clients and their families to empower them to manage their health and wellbeing.
- Maintaining accurate records and reporting on client progress and outcomes.
- A minimum of 1 year of experience as a registered nurse or nurse practitioner in a community-based setting.
- Demonstrated expertise in assessment, planning, and evaluation of client care.
- Strong communication and interpersonal skills, with the ability to build rapport with clients, carers, and healthcare professionals.
- Ability to work autonomously and as part of a team, with flexibility to adapt to changing priorities and circumstances.
- Current registration with the Australian Health Practitioners' Regulation Authority (AHPRA).
We offer a competitive salary package of $90,000 - $110,000 per annum, depending on experience, plus superannuation, paid leave, and career development opportunities.
